Forms & Documentation
Below are the forms users are required to complete prior to gaining access to Cognos or SARS. Once these forms are received a member of the ITS Application Services Team will process your request or will contact you should there be further questions regarding your request. Once processed, you will receive confirmation via email or phone call regarding system access. These forms may be submitted to our office via email at Helpdesk@tamusa.tamus.edu or by fax at (210) 784-1491. Although we accept these forms via email and fax, our preferred method of requests is through Agiloft, A&M-SA's Helpdesk Ticketing System.
