Self-Reporting Tool

All University employees and students must stay home and complete the COVID-19 Report Form if you answer “yes” to any of the questions listed below:

  • Do you have a diagnosis of COVID-19 (a laboratory confirmed case)?
  • Are you a suspected case of COVID-19?
  • Are you currently exhibiting symptoms associated with COVID-19 as they are listed on the CDC “Symptoms of Coronavirus” webpage?
  • Have you had close (less than six feet) contact with anyone that has a lab confirmed case of COVID-19 in the past 14 days?
  • Have you had close (less than six feet) contact with anyone who is awaiting results of a COVID-19 test within 14 days of exhibiting COVID-19 symptoms; and/or
  • Have you returned from travel or traveled through an area with state or local travel restrictions that mandate quarantine upon arrival home in the last 14 days?

STUDENTS should additionally notify the Dean of Students at dos@tamusa.edu or (210) 784-1354.

EMPLOYEES (including student employees) should additionally notify their supervisor.

SUPERVISORS must take the following actions when notified by an employee (faculty, staff, student employee and temporary) of any of the above-listed criteria:

  • Instruct the employee to go home immediately or stay home, practice self-isolation and complete the COVID-19 Report Form.
  • Inform the employee that the information provided through the COVID-19 Report Form will be used by the institution to provide them with information and resources and to initiate contact tracing, if necessary, and encourage the employee to cooperate with these efforts.
  • Immediately report any information provided by the employee through the COVID-19 Report Form (in case the employee does not do so in a timely manner).
  • Other than completion of the COVID-19 Report Form, supervisors should not relay any medical information about their employee to any other individuals.

FACULTY/STAFF should take the following action when notified by a student of any of the above-listed criteria:

  • Instruct the student to go home immediately or stay home, practice self-isolation, complete the COVID-19 Report Form and notify the Dean of Students (dos@tamusa.edu).
  • Inform the student that the information provided through the COVID-19 Report Form will be used by the institution to provide them with information and resources and to initiate contact tracing, if necessary, and encourage the student to cooperate with these efforts.
  • Immediately report any information provided by the student through the COVID-19 Report Form (in case the student does not do so in a timely manner).
  • Other than completion of the COVID-19 Report Form, faculty/staff should not relay any medical information about the student to any other persons.
