How to Request a Release of Information
In this video, you will learn how to sign an information release consent form in AIM.
There are two ways for a student to permit the release of information.
The first is during the application process.
Students can fill the form out before turning in the application.
If the student has finished the intake process,
students can request to release information by using the following steps.
Under my accommodations, select information release consents.
Choose New information release.
Specify the consent type,
then press, Select and Continue.
Fill out the consent form with the following information.
Contact person's full name, address,
phone number, relationship to student, e-mail.
Then answer the following two questions,
do you authorize Disability Support Services at Texas A&M University, San Antonio, two,
released to, obtain from, verbally exchanged with,
and documentation and information to be shared,
include and select all that apply.
Read the Information Release Consent Agreement.
Once it has been read, choose yes,
and then select "Submit Information Release Consent Form."
The file will be listed on the overview page.
This file can be viewed by student and DSS staff members.
